Niacinamide vs Azelaic Acid for Acne

Niacinamide and azelaic acid are two of the most well-tolerated topical ingredients for acne-prone skin, both useful, and they're often paired rather than chosen between. Understanding what each actually does helps you build a regimen that uses them appropriately rather than redundantly.
Niacinamide (vitamin B3) is a versatile supporting ingredient. Its evidence base includes: modest sebum reduction (about 25% with consistent use), anti-inflammatory effects on active acne, barrier strengthening (which helps tolerate other actives), reduced redness, and modest reduction in melanosome transfer (mild dark-spot benefit). It's available OTC in countless serums at 5-10% concentrations. It pairs with essentially everything without irritation issues.
Azelaic acid is more like a primary actor. Its evidence base is stronger for acne: prescription 15-20% azelaic acid (Finacea, Azelex) significantly reduces inflammatory lesion counts over 12-16 weeks, with effects comparable to lower-strength topical retinoids and topical antibiotics. It uniquely combines anti-inflammatory, antibacterial, and anti-pigmentary effects in a single ingredient, making it particularly useful for acne with significant post-inflammatory hyperpigmentation. It's safe during pregnancy and breastfeeding — a rare property among prescription acne treatments. Available OTC at 10% (lower potency) and by prescription at 15-20%.
For a comprehensive acne regimen, both have a place: niacinamide 5% serum AM (under moisturizer, as a daily supportive ingredient) plus prescription azelaic acid 15% PM (as a primary acne treatment). Add a nightly retinoid (tretinoin or adapalene) and a daily mineral SPF, and the regimen covers most concerns including pigmentation and barrier support.
For first-line acne, prescription azelaic acid + topical retinoid + benzoyl peroxide wash is a solid foundation. Niacinamide is an excellent add-on but doesn't need to replace any of those.
Why azelaic acid is pregnancy-safe when most acne treatments aren't
Azelaic acid is a naturally occurring dicarboxylic acid found in some grains; humans produce small amounts metabolically. Topical absorption is minimal and systemic exposure is negligible. Animal studies and decades of clinical use have shown no teratogenicity, leading to pregnancy category B classification (the safest category for medications without specific human pregnancy data).
This makes azelaic acid one of the very few prescription acne treatments safe during pregnancy and breastfeeding. Other options for pregnant patients are limited: topical erythromycin and topical clindamycin are also category B, but resistance has eroded their effectiveness. Benzoyl peroxide is generally considered safe but isn't formally classified. Most other acne treatments (tretinoin, tazarotene, isotretinoin, doxycycline, spironolactone) are contraindicated. Azelaic acid is the workhorse pregnancy-safe option.
